Recent developments have fans wondering where the tale will go next as Eiichiro Oda’s One Piece steadily draws to a close with the manga in its final epic. After beating Kaido at the conclusion of the Wano arc, Luffy’s final form, “Gear 5,” has been seen in the current Egghead Island storyline for the second time.
Even though it is obvious that Gear Five has incredible power, and some fans believe that the Hito Hito No Mi might be the series’ most potent fruit, there are still some unanswered mysteries. Given the impact earlier forms had on Luffy, many anticipate there would be consequences when the form is used, and this time it might be the most negative considering how potent it is.
Reduced Life
Many fans would be aware that Luffy’s lifespan was steadily shortened as a result of the previous Gears he used, particularly Gear Second and Gear Fourth. These forms were only utilized against equal opponents due to the physical strain they placed on the user’s body.
A brief scene during Luffy’s final confrontation with Kaido depicts Luffy losing strength and seeming very elderly and worn out in his basic form.
This may be a result of the drawn-out struggle with Kaido on Onigashima and the events leading up to it, but it may also indicate that Gear Five has placed the most physical strain on Luffy. This is quite comparable to Gear Third’s pre-time skip phase, which left Luffy in a baby form after utilising it, but this could be more dangerous.
Disease
Speaking of a life that was cut short, Gol D. Roger, a former “Pirate King,” was executed after turning himself in, but this was only possible because he was made aware of the illness that would finally claim his life.
Given the huge and obvious similarities between Luffy and Roger, it would not be surprising to see the same thing happen again. It is clear that Roger was not a Devil Fruit user, suggesting that the disease in question did not result from anything related to overusing his body for abilities.
Although it is unlikely that Luffy will experience this given the insinuation that he is “the one to surpass the Roger Pirates,” which predicts a different conclusion for Luffy and the other Straw Hats, it is still possible.
Because of Oda’s use of foreshadowing and the fact that character comparisons often signify more than people realize, it is plausible to assume that Luffy will be affected by the Devil Fruit’s qualities forever.

Reduced Gear Endurance
Luffy’s stamina was tested to the limit, as Kaido noted throughout their fight, and this was understandable given the earlier rounds of their fight and his run-in with Ulti before reaching the roof.
With Gear Fourth, Luffy still has trouble keeping his shape and, after powering down, is unable to use Haki for 10 minutes. This is obviously different from Gear Five because Luffy was able to quickly recover from exhaustion.
Oda hasn’t explicitly said or hinted that there are any significant negative effects after the employment of Gear Five since the conclusion of Wano and the confrontation between Luffy and Kaido, but those consequences are still very much a possibility.
This is also largely dependant on the circumstance and length of usage of the ability, and this was particularly true during the pre-time skip phase following Luffy’s battle with Lucci and his immobility.
Physical Stress in the Basic Form
Given that Luffy is now in top physical shape and that this is the concluding saga that many have been waiting for, this is more than likely not going to be a concern in the current stage of the plot, but it might very well catch up to an elderly Luffy.
One of the more obvious drawbacks would be a strain on Luffy physically if the story’s ending included any kind of time jumps to portray the Straw Hats as veterans years later.
Oda in One Piece has established a clear pattern that makes it clear that the old characters, such as Garp and Rayleigh, are not as strong as they once were. There is no way of knowing what effects Luffy’s increased usage of Gears, which are known to reduce his longevity, will have on his body in the future.
Changes to Physical Appearance
As demonstrated in his fights with Kaido and most recently Rob Lucci, Gear Five allows Luffy to fight without much restriction and in a free-flowing manner. This implies that Luffy can now be struck by devastating blows that cause his appearance to take the form of a weapon or demonstrate how he was struck.
While that is solely due to the characteristics of the Hito Hito No Mi Model Nika fruit and Luffy’s ability to alter the environment to his liking, it is possible that as a result, Luffy may appear much older than he actually is. This may explain why, after defeating Kaido, he appeared incredibly exhausted and more like an old man.
